Psychological Assessments & Diagnostic Evaluations

We provide comprehensive psychological evaluations using objective, evidence-based assessment methods to clarify diagnoses, guide treatment planning, evaluate attention and executive functioning concerns, and provide independent clinical opinions when appropriate.

Evaluations are tailored to the referral question and may be used for diagnostic clarification, ADHD assessment, and treatment recommendations.

These evaluations are designed to:

Clarify or confirm mental health diagnoses
Provide objective evidence beyond self-report
Address inconsistent records

Areas of evaluation may include:

Trauma and stress-related disorders
Mood and anxiety disorders
ADHD and attention concerns
Executive functioning difficulties
Diagnostic clarification
Treatment planning recommendations

Each assessment includes a structured clinical interview, standardized testing, and a detailed report with diagnostic impressions and clinical findings.

These services are evaluative in nature. The psychologist's role is to conduct an objective assessment and provide diagnostic impressions and recommendations. Assessment services do not constitute ongoing psychotherapy or treatment.

Supporting Veterans with Nexus Letters

In addition to therapy, Dr. Eubanks and her team offer professional nexus letters to support Veterans’ VA disability claims. These letters provide clear, evidence-based medical opinions connecting service-related experiences with current mental health conditions.

Two Options Available:

Nexus Letter Only – $750
Based on a comprehensive review of your medical records.

Comprehensive Nexus Package – $1200
Includes a psychodiagnostic evaluation, clinical interview(s), and professional medical opinion preparation when clinically appropriate.

Nexus services are completed and billed in phases, beginning with record review to determine clinical support for an opinion. Expedited services are available.
